Established in 1984, COFMC is committed to providing comprehensive primary health care, preventive health care education and outreach services at sites in Konawa, Prague and Wewoka.

Patients meeting the income guidelines for indigent care, based on salary and number of people in their household can receive primary care, dental and mental health services at a reduced fee.
